Our Home Health agency provides continuity of care between patients’ hospital stays and their medical care or services needed at home while maximizing recovery and promoting comfort of our patients whom we proudly serve.

Who is Eligible for Home Health?
Homebound individuals who are under the care of a physician requiring nursing care or therapy services.

Home Health Services is Covered by:

edicare, Medicare HMO, TennCare, Private Insurance, and Private Pay

Who may Refer a Patient for Home Health Care?
Doctors, Hospitals, Nursing Homes, Rehab Centers, Dialysis Centers, Healthcare Facilities, and Insurance Companies

All services provided without regard to race, color, creed, or national origin in accordance with Title VI of the Civil Rights Act of 1964.

Medicare’s Qualifying Home Health Skills

The following is a list of skills that most often qualifies patients to receive home health services covered by Medicare. If you feel that you need home health services to assist with your medical needs, you may talk to your physician about ordering an evaluation to determine if you qualify.

HOSPITALIZATION
When Medicare patients are hospitalized, they most often qualify for home health services.
